HETEROGENEITY: THE HOBGOBLIN OF
PREDICTING TREATMENT RESPONSE
CAN OUTCOME BE HOMOGENIZED?

by

Jonathan W. Stewart, M.D.
Division of Clinical Therapeutics
New York State Psychiatric Institute

Abstract

Because some improvement occurs due to specific treatment effects and other improvement is nonspecific "Response" or "Remission" must be heterogeneous. Therefore, studies trying to identify predictors of response must have low power. Dr. Stewart will explore ways to distinguish improvement likely due to treatment from nonspecific symptom change and the effects the resulting homogenization might have on identifying predictors of treatment outcome. Pattern analysis and on-off-on designs will be used as examples. Finally, Dr. Stewart will propose that rather than looking for "Predictors of Response" the question might better be rephrased "Identification of the biology required for SSRI responsivity.

Biographical Note

Jonathan W. Stewart received his M.D. at Yale University. After graduating medical school and completing his psychiatric residency at Hahnemann Hospital in Philadelphia in 1975, Dr. Stewart was an attending in-patient psychiatrist for three years before coming to the New York State Psychiatric Institute in 1978. Since then, Dr. Stewart has worked full time in the Depression Evaluation Service under the mentorship of Drs. Frederic M. Quitkin and Donald F. Klein. Since Dr. Quitkin's death in 2005, Dr. Stewart has shared Directorship of the DES with Dr. Patrick J. McGrath. Dr. Stewart's work has spanned biologic, antidepressant, psychotherapy and family studies. A major interest has been in identifying ways to produce more homogeneous populations as an aid to discovering the biology that will allow individualized treatment.
